This new and exciting opportunity, funded through the Rapid Rehousing Pathway (RRP) has been created to increase tenancy sustainment and move-on from supported accommodation for those accessing the dedicated Outreach Assistance Beds within Pathway 1, which is the commissioned male homeless provision within Bristol. We need you to help us make a difference to the homeless community; help shape and influence how we develop a positive long lasting solution for those who are rough sleeping. Key Responsibilities:
The post-holder will work within the Bristol homelessness male pathway with a fixed cohort or identified rough sleepers who require specialist intervention and multi-disciplinary approach to secure sustainable outcomes. These are in the following areas.
- Sustaining Accommodation
- Improve health and Wellbeing
- Engaging and sustain in formal treatment for those who have poor mental health; substance misuse; or learning difficulties
Working alongside local services, the rough sleeper navigators are responsible for carrying a caseload and supporting the day to day delivery of the programme. They are responsible for:
- Coordinating the transition from the streets or from supported accommodation into an independent tenancy for clients and being the single point of contact for this.
- Building and maintaining working relationships with agencies and individuals who can aid clients in sustaining their recovery and meeting these outcomes
- Delivering outcomes (as described above) for clients and evidencing their achievement
- Working with colleagues to address support needs, improving life skills and building recovery and building positive social circles (networks of support) with agencies, peers and from within the community to ensure clients sustain accommodation, improve health and take up work.
The successful candidate(s) will be able to demonstrate:
- Experience of delivering best practice in services for vulnerable people
- Experience of motivating complex and diverse client groups to sustain housing, health and or work outcomes and the necessary social skills to initiate and maintain constructive and appropriate relationships with clients agencies
- Proven track history of networking, building relationships and effective working in multi-agency context
- An ability to communicate effectively both verbally and in writing (English) and to collate and evidence work/outcomes using case management databases.
- Specialist understanding of those affected by rough sleeping
- An understanding services legislative environment regarding, housing and health, work and immigration as it relates to homeless people
